New Patient

Your first visit to Four Points Physical Therapy

Your first visit will include a thorough assessment followed by education on your condition.  We will then work with you to come up with a plan of care that suits your needs and goals. We will do our best to answer four key questions at this visit that will allow you to begin to take control of your problem:

  1. What is wrong with me?
  2. How long will it take?
  3. What can I do for it?
  4. What can you do for me?


Arrive

Please arrive 10-15 minutes prior to your first scheduled appointment to complete the necessary paperwork.


Bring

  • A list of your current medications, including dosage and frequency.
  • Your referral or prescription for physical therapy from your physician or health care provider.  Check with your health insurance company to verify if they require a referral or prescription. We are happy to assist you, if needed.
  • Relevant reports or medical records, if you have them.
  • Health insurance card(s)
  • Dress in comfortable, loose clothing or athletic wear.  Your therapist will need to evaluate and see the affected area.


Number of treatment sessions

How many treatment sessions you will need is a team decision between your therapist, your physician or heathcare provider and you. 

 

Insurance and Billing

We work with most major health insurance companies and networks, as well as Worker's Compensation and auto insurance. Be aware that if you are checking to see if we are in your network, we work through insurance companies as "Steamboat Spine and Sports Physical Therapy". Co-pays and co-insurance payments are due at the time of treatment.
